How to File Online

You will need the Unique ID printed on the Email or Postcard Notice that you received. If you did not receive a Notice or have lost yours, please contact the Settlement Administrator for assistance.

Please Note: Per the terms of the Settlement Agreement, you are a Class Member if you are a person in the United States or its territories. If you are not currently residing in the United Sates or its territories your claim will be denied. Please see FAQ 4 for further information or the Settlement Agreement for complete details.

How to File by Mail

If you wish to submit a Claim via standard mail, you may download a copy of the Claim Form here. You will need to provide all the information requested on the Claim Form, attach any supporting documentation, sign it, and date it. Then, mail it to the following address:

Remember, all Claim Forms must be submitted online or received by the Settlement Administrator no later than May 6, 2025.
